Web11 Sep 2024 · Heart: There was a right upper sternal thrill, but no parasternal lift. The rhythm was regular. The first and second heart sounds were soft. No extra heart sounds or clicks were appreciated. There was a grade 4/6 late-peaking systolic murmur noted at the left sternal border radiating to the base of the neck and into the carotids. Web21 Nov 2024 · examination reveals a left parasternal lift, loud P2, and right-sided S3 and S4. There are no audible murmurs. CXR reveals clear lung fields and an ECG shows evidence of right ventricular hypertrophy. Pulmonary function tests show a slight restrictive pattern. Primary pulmonary hypertension is diagnosed.
